F&M Healthcare Services offers PASSPORT (Pre-Admission Screening Providing Options and Resources Today) Services to all eligible seniors. PASSPORT is a Medicaid Waiver program that offers seniors with additional services so that they can continue living in their own home.
Private Duty Passport Services includes, but is not limited to:

- Home Delivered Meals
- Adult Day Services (Adult Day Care)
- Emergency Response System
- Respite Care
- Specialty Consults
- Transportation
- Minor Home Modifications
- Chore Service and Other Services
